Membership Categories and applications

Active Membership

Active members are fully qualified urologists who reside or practice in the Illinois, Lake and Porter County Indiana.

To apply for active membership, please forward the following materials to the Central Office:

Application
• An updated copy of your CV
• Two letters of recommendation from Active CUS members
• A $600 payment that will be applied towards your first year's dues (includes: membership, meeting registration, scientific program and dinner)

Associate Membership

Associate members are interested individuals residing outside of Illinois, Lake and Porter County Indiana.

To apply for associate membership, please forward the following materials to the Central Office:

Application
• An updated copy of your CV
• Two letters of recommendation from Active CUS members
• A $275 payment that will be applied towards your first year's dues (includes: membership, meeting registration and scientific program)
