Active Membership
Active members are fully qualified urologists who reside or practice in Illinois, Lake, and Porter County Indiana. To apply for active membership, please forward the following materials to the CUS office info@cuschicago.com.
An updated copy of your CV
Two letters of recommendation from Active CUS members
A $600 payment that will be applied towards your first year's dues (includes: membership, meeting registration, scientific program, CME credits, and networking reception)
Senior membership
Qualified urologists residing and/or practicing in the Chicago area and over 65 years of age.
Pay dues, complimentary meeting registration, and receive CME credits.
emeritus membership
Fully retired from the practice of urology and over 65 years of age.
Register for the occasional meeting attended.
Status is approved by Board of Directors.
